If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. We ideally require 6-8 weeks’ notice in order to book your travel vaccination appointment. You will need to complete a travel assessment form (available from our website or our treatment room reception) and leave it with us for 2 weeks before phoning to make your appointment. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.
WE ARE UNABLE TO OFFER last minute appointments.
There is currently a SHORTAGE of SOME travel Vaccinations. We may unfortunately be unable to offer you the vaccinations you require, however you may be able to obtain these privately from some pharmacies who have some stock remaining. We have been advised this shortage may continue into 2018.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel. The practice ask that you download and print this form and hand into reception before you make the initial appointment - this will enable the travel nurse to ensure she had the correct information in front of her at your initial consultation appointment.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
