CORONAVIRUS (COVID-19) GUIDANCE

If you suspect you have symptoms of coronavirus, do not come to the surgery. Self-isolate and access 111 online for guidance on your particular circumstances. 

You do not need to contact NHS 111 to tell them you're staying at home.

Stay at home if you have either:

  • a high temperature
  • a new continuous cough

This will help protect others in your community while you are infectious.

Current official guidance from Public Health England is to stay indoors and avoid contact with other people.

In the last 14 days, if have you travelled to the UK from:

 or:

  • you have returned from Italy since 9th March 2020

Stay indoors and avoid contact with other people (even if you have no symptoms). To find out what to do search 'nhs coronavirus' for advice or access 111 online coronavirus service.

if you’ve travelled to the UK from the following places in the last 14 days and have a cough, high temperature or shortness of breath, even if your symptoms are mild:

  • mainland China outside of Hubei province
  • South Korea outside of the special care zones
  • Cambodia
  • Hong Kong
  • Japan
  • Laos
  • Macau
  • Malaysia
  • Myanmar
  • Singapore
  • Taiwan
  • Thailand
  • Vietnam

Do not go to a GP surgery, pharmacy or hospital.

New Patient Registration

You must reside within our practice area to be eligible to register (this can be found on our practice leaflet). You will need to present at reception to either fill in our forms or hand in ones printed from our website (see links below).

Please bring photo ID and proof of address with you when you register.

If we require further information to enable us to complete your registration with us we will contact you within 14 days of receipt of your forms, so please ensure we have a telephone number. If we are unable to contact you within this period your registration may be cancelled and you would be required to complete the registration forms again.

New Patient Forms

New patients are required to fill in the following three forms to in order to register:

1.) GMS1 form (for the Health Authority)

GMS1 form

2.) New Surgery/Brambles form - please select from:

Age 16 and over

Age 1-15 inclusive

New babies

3.) 

Alcohol screening questionnaire

Please complete all registration forms and bring in to either surgery:

The New Surgery, 8 Shenfield Road, Brentwood, Essex CM15 8AB

The Brambles, Greenwich Avenue, Geary Drive, Brentwood, Essex CM15 9DY (open Monday, Tuesday and Thursday only)

New registrations can only be carried out face-to-face.

If you are registering with the practice and are between the ages of 40 – 74 years and without any previous medical history of note( i.e. no diabetes, cardiovascular disease, etc.) we would like you to undertake an NHS health check as part of your registration.

Please do not worry, as the results of this check do not mean that we will not register you.

The health check has been designed for patients in the age cohort to help prevent cardiovascular disease from developing.
