Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to take the following steps:

Your travel form needs to be submitted 6 – 8 weeks prior to your departure date. You will need to complete this (click link to open form) Travel Assessment Form and hand this into Reception. You will then receive a phone call or text message as soon as the assessment form has been processed by our Practice Nurses.

WE ARE UNABLE TO OFFER LAST MINUTE APPOINTMENTS.

If your departure date is less than 4 weeks we may not be able to process your assessment form and you may be advised to attend a private travel clinic in order to receive your travel vaccinations.

Please be aware that some travel vaccines are ordered on a private prescription, and thus incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
